Simple Survey is a fully configurable survey block supporting a wide range of question types: single and multiple choice, true/false, dropdowns, numeric inputs, free text, Likert scale grids, matching pairs, section labels, and pill-style toggle selectors. Questions can be grouped into pages with flexible navigation options, and an optional introduction page can carry a heading, body text, and image to set context before the survey begins. Responses can be tracked automatically as SCORM/xAPI interactions on submit, and/or posted as JSON to any external URL. Comes with a set of prebuilt colour themes that can be overridden, making it straightforward to match any course design.
